NON-DISCRIMINATION POLICY
Quadrant Virtual Care Management LLC and its affiliates and managed entities, “As You Are,” does not exclude, deny benefits to, or otherwise discriminate against any person on the grounds of race, color, national origin, or on the basis of disability or age in admission to, participation in, or receipt of the services and benefits of any of its programs and activities or in employment therein, whether carried out by As You Are directly or through a contractor or any other entity with whom As You Are arranges to carry out its programs and activities.
As You Are provides diagnostic services to children between the ages of 16 months to 10 years. This qualifying age range has been established to provide the most medically appropriate services, and is not intended to discriminate against any individual on the basis of age.

COMMUNICATIONS WITH PERSONS WITH SENSORY IMPAIRMENTS
As You Are is committed to ensuring that qualified persons with disabilities, including those with impaired sensory or speaking skills, receive effective notice concerning benefits or services or written material concerning waivers of rights or consent to treatment. All aids needed to provide this notice are provided without cost to the person being served.

COMMUNICATION WITH PERSONS WITH LIMITED ENGLISH PROFICIENCY
As You Are is committed to supporting those with Limited English Proficiency by providing appropriate communication aids for services not carried out by As You Are directly, to ensure a meaningful opportunity to benefit from services for the diagnosis of Autism.

NOTICE OF PROGRAM ACCESSIBILITY
As You Are and and all of its programs and activities are accessible to and usable by disabled persons, including persons with impaired hearing and vision.
Access features include a full range of assistive and communication aids provided to persons with impaired hearing, vision, speech, or manual skills, without additional charge for such aids: They will be arranged for virtual communication if and when needed.

If you believe that As You Are has engaged in discriminatory activity on the basis of race, color, national origin, age, disability, or sex, you can file a grievance with the ACA Section 1557 Coordinator. You can file a grievance in person or by mail, fax, or email in accordance with the Plan’s grievance procedures (a copy of which may be obtained from the ACA Section 1557 Coordinator).
