New Patient Evaluation

First-time evaluation for children 16 months to 10 years old who haven’t received an autism diagnosis yet. Our comprehensive three-part assessment provides clarity and guidance for your family’s journey. Referrals are not required to get started.

Patients up to 10 year
old recertificationre

Updated Re-Evaluation for children up to 10 years old who have previously received an autism diagnosis from As You Are or another provider. Maintain access to services and accommodations with current documentation.

Patients older than 
11 old recertification

A Re-Evaluation for kids and teens 11 to 17 years old. Keep therapeutic services and educational supports in place with a timely re-evaluation during important developmental transitions.

Requirements:

  • Previous diagnosis for autism through another provider
  • Documentation of previous autism diagnosis through another provider

Returning Patience

Follow-up appointments for existing As You Are patients. Request appointments for follow-up care or care coordination by sending us a message in your patient portal. Connect with your care team to discuss progress, address new concerns, or update your care plan as your child develops.

details with your child’s physician.

  • Parent/guardian/caregiver ID
  • Previous evaluation reports

How can I access my appointment?

You’ll receive an email or text (if you’ve

opted in) one day before your appointment

with your visit link and instructions. We recommend testing your device beforehand.

*As part of our process, we will verify your child’s insurance coverage and potential out of pocket responsibility. We will provide an estimate to you via email before your child’s first scheduled appointment. If your insurance is out of network, you will provided additional payment options for moving forward. Prior to scheduling, depending on the insurance information provided during intake, a credit card on file may be required to proceed with appointments.

A note to families

Our goal is to ensure every child gets timely, appropriate care. However, As You Are’s

virtual autism evaluation isn’t right for every situation. If your child experiences any

of the challenges below, we recommend an in-person evaluation

  • Significant history of trauma or abuse
  • Strong concerns that the child may be masking symptoms in certain environments
  • Motor or cognitive delays that prevent following simple 1–2 step movements (e.g., throwing a ball, pretend play)
  • Oppositional or defiant behavior that would limit participation in a virtual play-based assessment
  • Two or more existing psychiatric conditions 
(e.g., ODD + anxiety, ADHD + DMDD)
  • Significant hearing or vision impairments
