It’s okay to wonder if your child is hitting important milestones as they grow. We know that every child is unique and we’re here to help you on this journey, wherever that is and whatever that looks like for your family.

STEP 1 – Scheduling your child’s first appointment

What you can expect:

We’ll need some specific health information about your kiddo to pass along to your dedicated physician prior to your appointment. We’re going to ask you to answer a short questionnaires that will be provided to your child’s dedicated physician. These can take 5-10 minutes to complete. Here are some things to have on hand for scheduling:

  1. Child’s Insurance Card
  2. Parent, Guardian or Caregiver ID
  3. Any previous evaluations your child has received

STEP 2 – Complete your medical history paperwork for the physician

My appointment has been scheduled, what’s next?

Once you schedule your appointment, you’ll receive a secure link to start working on additional paperwork that should be completed as far in advance to your appointment as possible. You will not be able to start your first appointment without completing the paperwork link. Don’t worry, if at any time you need to step away from your device while inputting information, you can pick up right where you left off using that same secure link.

STEP 3 – Test your device and join the appointment

How can I access my appointment?

One day prior to your appointment you will receive an email or text (must opt-in on the paperwork), which includes your appointment link and detailed instructions for your appointment. We recommend you test your device in advance, click here.
