It’s okay to question whether your child is developmentally progressing in-line with key developmental milestones. Remember, these are just markers, and every child is unique. We’re here to help you and your family navigate!

How It Works

Provider refers a family to As You Are and the family receives an email with instructions on how to get started.
OR
Parent or Guardian visits AsYouAre.com.
Family provides basic information like family history, development and health history using the online questionnaire.
Family schedules the 1st of 3 appointments with their dedicated physician.
During the results (3rd) appointment, physician shares diagnosis with family for appropriate care.
As You Are Care Sidekicks™ are available to connect families with appropriate resources and guide the family along their journey after a diagnosis.

Things to Know:

Scheduling a virtual evaluation appointment is just like scheduling a visit with any doctor, and the best part is that the entire process can be done from the comfort of your home!

Who should schedule the appointment?

This should be completed by the child’s parent or guardian.

What information will you ask me for?

We’re going to ask you to answer a short questionnaire known as the MCHAT or ASDQ (based on the child’s age) and a handful of questions that will be provided to your child’s dedicated physician. You should be prepared to provide: 

  • Child’s Health Information
  • Guardian Information
  • Preferred method of payment 
    • If you’d like to use insurance, we need a picture of your insurance card
  • Contact information
  • Consent forms

Why do you need that information?

This helps us understand your child’s likelihood of autism and will help your designated physician learn more about your child ahead of the first virtual appointment.

Why do you need my payment information now?

Whether using self-pay or insurance, we will  ask for your credit card to be put on file. Please note that no charges will occur until after your child’s first appointment with a physician. 

If using insurance, our team will conduct a full benefits investigation on your behalf, at no cost. We do require your insurance card information, image and consent. This process could take between 3-5 business days, and our team will review with the parent or guardian via phone prior to the first visit with a physician. Each patient’s responsibility is specific to his or her health insurance plan (due to differences in copays, coinsurance and deductibles).  A friendly reminder – we cannot process a benefits investigation without the appropriate information and consents. This could postpone any potential appointment.

What happens after I provide this information?

It’s time to get that appointment scheduled! You will have the flexibility to select dates and times that work best for you and your family. 

Once you schedule your appointment, we’ll send you a secure link to start working on the paperwork we mentioned above.  We recommend you complete the paperwork sooner than later, and if at any time you need to step away from your device while inputting information, you can pick up right where you left off using that same secure link. As your child’s appointment gets closer, our team will reach out with a friendly reminders from “As You Are” via email and text (if you opt in on your paperwork). These notifications will have important details about your upcoming appointment.
